We are excited to have you join us! Below are a few terms we ask you to review and agree to before reserving your spot(s).

Fee: $40 per person (Non-refundable)

This agreement is entered into between the undersigned participant (“Participant”) and BURN, Ventura Lagree LLC (“BURN”), for participation in a sound bath wellness event at the BURN studio.

Event Date: Sept. 7, 2025

1177 E. Thompson Blvd., Suite 102 B, Ventura, CA 93001

By signing below, the Participant agrees to the following:

I understand that this event includes meditative exposure to sound instruments such as crystal singing bowls, chimes, gongs, and similar. I recognize this is a voluntary wellness experience and not a medical treatment.

I acknowledge that individual experiences during sound baths may vary and, while typically relaxing, may produce emotional or physical reactions. I accept all risks associated with my participation.

I release and hold harmless BURN, its owners, instructors, affiliates, and the property owners—including Sherri Silva—from any liability for injuries, losses, or damages sustained in connection with the event, whether caused by negligence or otherwise.

I certify that I am in suitable physical and mental condition to participate. I understand the $40 event fee is non-refundable.

I acknowledge that all attendees must bring their own yoga mat. A blanket, pillow, or other comfort item is optional and encouraged. No mats or props will be provided.

For the comfort and safety of all attendees, I understand that no children or pets are permitted at the event.

By checking the box at check out and putting my name(s) to the agreement, I affirm that I have read and understand this agreement and voluntarily accept its terms.
